693filler provides a local desktop tool and related website services for Form I-693 vaccination record workflows. It is not affiliated with, endorsed by, or sponsored by USCIS, DHS, CDC, or any government agency.

Purpose and eligibility

The document-preparation functionality is intended for USCIS-designated civil surgeons and authorized personnel working within their practices. It is not offered as a consumer self-filing tool. You must be legally able to agree to these terms and authorized to use every document you process.

Applicants complete Part 1 and identifying information as directed by USCIS. The civil surgeon is responsible for reviewing vaccination documentation, making medical determinations, completing Part 10 and the other required professional portions, and following the current USCIS and CDC instructions.

The service does not provide legal, immigration, medical, clinical, or civil-surgeon advice. It does not determine vaccine requirements, medical eligibility, immigration eligibility, filing strategy, or whether any form is acceptable to USCIS.

Review of results

Generated PDFs may be incomplete, inaccurate, outdated, or unsuitable for filing. You are responsible for reviewing every field, confirming the current USCIS form edition and instructions, and obtaining review from the appropriate qualified professional before use.

You are responsible for the legality, accuracy, completeness, consent, and authorization of all files and data you upload. Do not upload records unless you have the right to process them through this service.

Prior web requests and reruns

New patient-document processing through the public website is no longer offered. Existing payment confirmations, owned downloads, and eligible reruns remain available for requests that were prepared before that workflow was retired.

Except where required by applicable law, prior web payments are final and non-refundable. If a verified service failure affects a paid order, you may use one rerun during the stated eligibility period with the exact same vaccination PDF files. A rerun does not cover changed files, replacement records, a different person, or a change of mind.

If you select email delivery, you authorize 693filler.com to send the completed PDF to the address you confirm. You are responsible for entering an address you control or are authorized to use and for the privacy and security of the receiving mailbox. Delivery is complete when the outgoing email service accepts the message, although inbox delivery may be delayed or filtered. When available, the short-lived browser download remains an additional way to obtain the PDF.

Delivery support may use the exact email address or order reference you provide to locate a limited support record for up to 90 days. The record contains a protected email match, masked address, anonymous order and delivery status, and related times, not a readable stored address, applicant identity, vaccination details, or PDF contents.

Keep your order reference and optional rerun code until you have reviewed the downloads. Rerun codes expire, can be used only once, and do not guarantee that a submitted form will be accepted by USCIS or another party.

Desktop access

Desktop access uses prepaid form credits or an invited pilot program. Unless checkout states otherwise, one credit permits one completed Form I-693 PDF. Each credit-pack reload requires your approval and is not charged automatically. Pilot access is free only during the assigned dates. Eligible pilot installations receive the displayed reduced 100-form price for 12 months after the pilot ends, after which standard pricing applies.

Checkout is hosted by Stripe. Stripe may offer Link or another option to save payment details under its own terms and privacy notice. 693filler does not receive or store full card numbers or security codes.

A valid desktop entitlement permits the purchaser and its authorized users to install and use the desktop software for its intended internal purpose. Access is limited, revocable, non-exclusive, and non-transferable except where a written agreement or applicable law states otherwise. Desktop access checks send only limited account and usage information and do not authorize 693filler to receive patient documents.

The software, workflow, design, and original content are owned by Vindem Labs LLC or its licensors. Access does not grant permission to copy, republish, resell, redistribute, bypass license controls, or reverse engineer protected portions of the software, except to the extent applicable law does not permit that restriction.

Responsibility and liability

The desktop edition is designed for local document processing, but it does not make a person or organization compliant with HIPAA or any other legal, privacy, or security standard. Organizations remain responsible for their devices, user access, backups, policies, risk assessment, training, and any required agreements.

To the maximum extent permitted by law, the service provider disclaims liability for filing decisions, medical determinations, immigration outcomes, rejected forms, data entry errors, uploaded content, or reliance on generated documents.
